"Oestrogen dominance" is used to explain everything from bloating and mood swings to weight gain and heavy periods. But is it a real medical condition, or a catch-all label that oversimplifies a complex picture?
The honest answer is somewhere in between. Oestrogen dominance is not a formal diagnosis used by most endocrinologists or gynaecologists - you will not find it in medical textbooks in the same way you will find PCOS or hypothyroidism. But the concept it describes is a real hormonal pattern that does affect a significant number of women and does cause real symptoms.
Understanding the biology behind it, what actually causes it, and what can genuinely be done about it is far more useful than either dismissing it entirely or accepting it as an explanation for every symptom.

Oestrogen and progesterone work together across the menstrual cycle. Oestrogen dominates the first half of the cycle, driving the growth of the uterine lining and the development of follicles. After ovulation, progesterone rises to balance oestrogen; it stabilises the lining, supports implantation, and has a calming effect on the nervous system.
The problem arises when this balance is off. Oestrogen can be excessive relative to progesterone in two ways:

The liver breaks down oestrogen into metabolites that are then cleared through the gut. When liver function is impaired, oestrogen clearance slows, and levels build up.
Gut health also matters here. The gut microbiome (through bacteria) influences how oestrogen is processed. Dysbiosis can allow oestrogen to be reactivated and reabsorbed rather than excreted.
Certain environmental compounds mimic oestrogen in the body. These are found in plastics, pesticide residues on food, some personal care products, and non-stick cookware. The effect of any individual exposure is small, but cumulative exposure over years is thought to contribute to hormonal imbalance in women. A high-fibre diet supports oestrogen clearance through the gut. Cruciferous vegetables like broccoli, cauliflower, cabbage, and kale contain compounds that support liver metabolism of oestrogen. Reducing refined carbohydrates and sugar improves insulin sensitivity.
Reducing abdominal fat decreases aromatase activity and reduces the extra oestrogen produced outside the ovaries.
Yoga, pranayama, adequate sleep, and reducing chronic stress protect progesterone levels by keeping cortisol in check.
Limiting alcohol intake, staying hydrated, and eating liver-supporting foods help support oestrogen clearance.
When blood tests confirm low progesterone or elevated oestrogen, a gynaecologist may prescribe progesterone supplementation in the second half of the cycle. This is particularly used in women with luteal phase deficiency, recurrent early miscarriage, or severe PMS symptoms.

Estrogen dominance is not a myth, but it is also not a diagnosis that should be self-assigned based on a symptom checklist. The concept describes a real hormonal pattern and the symptoms it causes are genuine. But the causes differ, and management depends on what is actually driving the imbalance.
