When to see doctor for Irregular Periods? | Menorrhagia, Oligomenorrhea & more | Dr. Manjula Deepak
"To see a doctor for irregular cycles we have to understand what is irregular cycle. There are a lot of variation in the normal cycle . Normal cycle ranges from 21 to 35 days. Woman having 21 to 35 days cycle for 4 to 5 days in the cycle is considered as normal. If you skip for 4 to 5 days it is still considered as normal. So when it deviates from the normal, when you are having normal cycles according to your cycles but for the last 3 months you are seeing any regular cycles. So for the last 3 months you see any irregular cycles. So atleast 3 moths you need to maintain your calendar. If your cycles goes less than 21 days that is you are hag frequent cycles or you are having frequent cycles that is more than 35 days that is infrequent cycles, that is when we think there is some deviation. third thing is when the bleeding is excessive, that is more than the normal 5 to 6 days that is more than 8 days and there are lot of clots and tissues passing in the first 2 days of your cycles, and you see clotting even after 4th or 5th day and you see some tissue s coming out even on the 4th or 5th day, of your menstruation is painful that persists for more than 4th days or 5 the day and I also told there is a condition called as hypomenorrhea, that is your cycles stay only for first days or second day. you have regular cycles but you see only spots for 1 or 2 days that is hypomenorrhea. In menorrhagia you have cycles for more than 8 days. Third is oligomenrrhea, that is once in more than 35 days, like 1 month skip, 2 month skip or 3 month skip. Fourth is intermenstrual bleeding, in between your cycles you see spotting or bleeding. Fifth is polymenorrhagia is where your cycles are excessive and it comes in once in less than 21 days and all these condition you have to visit a doctor and all these persist for more than 3 months of cycles and it is better to go for investigation"
Cause of Irregular Periods | Menstrual Irregularity | Hormonal Cause & More I Dr. Manjula Deepak
"To have regular cycles we have something called hypothalamus which is in the brain, ovaries which is responding to your uterine axis. There are hormones secreted in the hypothalamus which is called as GnRH hormone which goes into the pituitary gland to produce a hormone called as FSH and LH. This will act on the ovaries to produce the main two hormones called as estrogen and progesterone. These are responsible for regular ovulation and this causes regular bleeding. Whenever there is pathology in the hypothalamus or pituitary or ovarian cause. This causes irregular cycles. So the irregular cycles causes hypothalmo pituitary axis, that is as simple as stress related or going through anxiety period or nutritional imbalance, like excessive eating or vigorous exercise. Can be as simple as antihypertensive or antipsychotics or tumors in the brain. Second coming into thyroid gland. Thryoid abnormalities can cause a lot of irregular cycles. Coming to the ovaries, ovarian tumors, commonest cause being endometriotic cyst. Uterine abnormality like fibroid uterus, endometriosis or adenomyosis uterus or any tumors or common in Indian scenario called as tuberculosis uterus or vaginal infections, vaginal septum, absence of the uterus in vagina, all these conditions can cause these conditions"
Normal Vs Abnormal Periods| Is your Monthly Cycle Irregular-Signs & Symptoms I Dr. Manjula Deepak
"What is regular periods? Every woman will have a variation in their periods. Normally we call it in a range of 21 to 35 days and bleeding for about 5 to 6 days is considered as normal. Any deviation from this scenario is considered as irregular period. If a woman has less than, for every 21 days or she gets more than 35 days, then it is considered a s irregular period or the frequency and the amount of the bleeding generally we have for 5 days to 8 days of bleeding, when the bleeding goes beyond 8 days and it is excessive generally the first 2 days of bleeding it is excessive, can be associated with a little bit of clots, but if the clots continue for more than 5th day of the cycle and you see tissues continuing for more than 4 days, then this is the scenario where you have to considered as irregular period. Irregular period has many names, one is oligomenorrhea, that is if your cycles go more than 35 days, amenorrhea is when you don’ t have cycles for more than 6 months, hypomenorrhea is when your cycles have presented for juts 1st day or maximum 2nd day. Or you just have sporting. Third is menorrhagia, is when your cycles is heavier or clotting or tissue passing for more than 3 rd day. Or intermenstrual bleeding in between your cycles is when you have regular cycles. All this is considered as regular and also most important in married woman is post coital bleeding. If this persists for 3 cycles, you need to visit your doctor"
Irregular Periods & Pregnancy Chances - Causes | TIPS TO GET PREGNANT FASTI Dr. Manjula Deepak
"Irregular periods does cause difficulty in conception. There are various causes for irregular periods. To have regular periods, there is a HPO axis, which is a Hypothalamo pituitary ovarian axis. This is related form the brain to the ovaries then to the uterus, then the bleeding occurs. Any causes which will lead to irregular periods, which can be a Hypothalamic cause or an ovarian cause or a uterine cause which can lead to irregular periods. It becomes very important for us to know whether the irregular periods is just due to a physiological condition or a pathological condition. As long as we know it is a physiological condition, you need not worry that it can lead to pregnancy difficulty. There are various causes for physiological condition. It can be a stress or drug induced or nutritional defect or vigorous exercise. In these conditions you need not worry that there is any issue with the pregnancy but there are certain pathological conditions like a tumor in the brain or an abnormality in the uterus, where there is a fibroid or uterine tuberculosis or an ovarian cause which is the commonest being polycystic ovarian disease or endometriotic cyst. It can also have a defect in the vagina where here can be absence of vaginal septas. In these conditions there can be irregular periods. In that condition there is a difficulty in conceiving and you need to require to take guidance from your obstetrician"
High Blood Pressure in Pregnancy: Effects on Mother & Baby | Preeclampsia | Dr. Brunda Channappa
"Preeclampsia if it happens at the time of maturity, say when the pregnancy is termed beyond 34 weeks, then we deliver the mother and the baby and that should be a happy ending interms of the mother being well and the being well. Preeclampsia, I hope you understand when the mother develops high blood pressure in pregnancy and it typically happens in 20 weeks of pregnancy. So if this happens very early in pregnancy then we can extend the pregnancy to the extend we want because we want the baby to be matured when it is delivered ad we want the baby to survive on its own when it is delivered, we want to avoid the immaturities we can. There are conditions we have where the condition of the mother gets serious and we don’t want to continue the pregnancy, because counting pregnancy meaning risking the others left and at that time we are forced to deliver because ultimately the treatment of Preeclampsia is to remove the placenta, which is the trigger for the blood pressure. So to remove the placentas, we have to reeve the baby, no matter what stage the pregnancy is , if the mother’s life is at risks of the outcome of the pregnancy, the risk to the mother depends on the stage of pregnancy that she is if she is quite close to delivery, we will deliver her and not take any major risk with the mother’s life. But if she is so far from the delivery, then it means every day inside the uterus means 2 days outside the NICU, then we will try to push the pregnancy forward. So there will be a risk to the mother’s life, that we will be taking to a certain extend and similarly the baby who comes out early will go through a lot of struggle in the NICU and if it happens even before the baby is viable. The baby will not survive. So these are the risk of Preeclampsia on the other and the baby. This is one of the major worries of any mother during the pregnancy. High blood pressure in the mother also doesn’t allow the baby to grow well. So these babies are growth restricted, growth retreaded and they may run into trouble as well even though the mother is stable requiring delivery of the baby for its well being. So Preeclampsia is a delicate scenario that needs to be monitored and described on again individual basis"
Birth defects detected before Child's Birth | Adverse Fetal Outcome I Dr. Brunda Channappa
"Not all adverse fetal outcomes can be detected before childbirth. Some can be detected. We do testing for thyroid, sugars so that we prevent these adverse fetal outcomes. If we know a preexisting problem in the mothers then we know there is a problem to look out for certain conditions, say for example, if a mother develops a viral infection like rubella, there are tests to see if the baby is affected and that will then predict if the baby is going to have a problem after it is being born or not. That is a rare incident. Routinely we do scans at 12 week and 5 months to check if the babies are normal. This is one of the main ways to check what is the outcome of the baby is going to be at 12 weeks we are going to do a Down’s syndrome screening, scan and blood test to see what the baby to see if the baby is normal with regards to Down’s syndrome. Again at 18 to 20 weeks, we do scan which we call as anomaly scan were we check to see if the baby is looking anatomically normal. Through our scanning doctor will do a whole lot of scans to see if the baby is normal, not all abnormalities are present at that time sometimes we see that affect the baby is growing there are some things that are not quite right as the baby is growing, but for the most part, most problems are detected, by the 5th month, and most of the tests are detected by the way of ultrasound scans"
