"PCOS people once they get pregnant, maybe they would have got pregnant naturally, they have oligoovulation , that is once in a while they ovulate or they may have conceived with ovulation induction or with IVF or IUI but once they get pregnant we can’t relax. We have to know that they have insulin resistance and hormone imbalance the may lead to more of miscarriages and they are prone to insulin resistance and they may develop diabetes in pregnancy because they are obese and they have hyperlipidemia they have hypertension in pregnancy, because of all these problems they lead to IUGR or pregnancy and because of GDM they lead to macrosomias"

"PCOS as discussed hampers fertility in the reproductive age group and for people who are trying for pregnancy they come initially for trying them for 1 year then we start them on treatment. Initially it causes problem by not forming the egg. Once the egg from sometimes it doesn’t rupture. If the egg doesn’t rupture then we cannot have a pregnancy serial ultrasounds where we start the on follicular monitoring, we give tablets flowed by insulin sensitizers like metformin, and some folic acid tablets those who don’t respond to tablets need to go in for hormonal injections, HMG, FSH and with this we further along with this we have to go for IVF some people. Also try with laparoscopic ovarian drilling, which can be done a s a treatment modality for PCOS. Ovarian Drilling is a surgical method and it has to be adopted in the end, after they don’t respond to tablets or infections because many at times they loose a lot of eggs during Polycystic Ovary Drilling. If the drilling is done in a wrong way then they may end up in ovarian failure. So I advice them to is advise them to try with tablets and injections not responding to that then go for surgical method"

"The Treatment modalities available for PCOS is everybody’s concern because the urban population almost 50% of the population is ending up in PCOS right form teenage, to reproductive age and menopausal age. So according to age wise you have the treatment for PCOS. In teenage all I would advise is to have a healthy eating, exercise habits. If the PCOS is to bad and have cycles once in 6 months , we go in for oral contraceptive pills. Which is a cyclical pills and we use it as a treatment modality. Coming to reproductive age, we have to see if the woman wants to conceive or to avoid pregnancy right now if the want to avoid pregnancy the polycystic ovaries will be treated with contraceptive pills whereas people wanting pregnancy. For them we want ovulation caution and if they dont respond to tablets, then eg for IUI , then their last stage will be contraceptive pills. So among all these stages the continuous exercise have to be continued then the result of this has to be continued. Even the women in menopausal age they have irregular heavy bleeding and people having PCOS they will be having more to estrogen lifelong and will have heavy menstrual bleeding lifelong and they will have higher risk of endometrial hyperplasia, endometrial cancer and breast cancers. So they need to be deviated of the same. So healthy lifestyle is what I would advise everybody for PCOS"
