COVID test in Newborn | When and How? I Dr. Arjun S K
We need to do covid tests in the newborn also. If the mother is tested positive or suspected to have covid then we need to do the covid tests in the newborn. The second scenario is when there is a cluster outbreak. That means any body in the family or anybody staying with the baby got infected thrid is when the mothers covid status is unknown and suddenly the newborn comes to the NICU, with few symptoms like hurried breathing, fever. So in these cases we need to tests for COIVID. So the re is a gold standard test, what we do is RTPCR . the tests includes the nasal or the oral swab. So we take 24 or 48 hours after birth and we do the test.
Can we get children vaccinated against Covid -19 soon? I Dr. Nilesh Rao
"There is ongoing research worldwide regarding vaccination in children and there are many studies which are in various phases. However we can expect vaccines by this year end or early 2022. We now the Indian vaccine covaxin trials are ongoing for 12 to 18 year old kids and also for 6 to 12 year old kids so we will wait for the results. The covishiled vaccines trial started in February 2021 , but has been halted due to various reasons. So currently in the united states the older kids are being vaccinated with the mRNA vaccines. There is also a new vaccine which is a recombinant nano particle protein based vaccine which is called as covovax which may be available in India by the year end"
Tips to protect CHILDREN from COVID 3rd Wave | Dr. Nilesh Rao
"As clearly outlined by the Indian academy of pediatrics guidelines for the 3rd wave, there is clearly no reasons to panic. As we know in the 2nd wave family clustering was one of the common scenario that we were commonly seeing wherein the adults used to bring the infection and subsequently the whole family was infected. So it makes logical sense that the immunization of adults is the most important measure to protect your child who is at home it is also important to understand that your child if infected may transmit the infection in the household contacts and so getting all the seniors vaccinated non priority basis is important. Children above 2 to 5 years can be trained to wear the mask properly and the adults should be the role models for the kids with respect to hand hygiene, social distancing norms and mask etiquettes"
Can my Newly Born Baby Get Infected from Me? I Dr. Arjun S K
The newborn can get infected with covid in 3 modes, one is before birth or during birth or one is after birth . what we call in medical terms is intrauterine infection or intrapartum during the time of birth or post partum or after the delivery. So usually what will happen is intrauterine in the sense from the placenta the virus can pass from the blood vessels from the mother to the baby and that can infect the baby and we also have reports that suggests that the liquids that surround the baby inside the womb, it can also contain viral particles normally baby keeps swallowing and inhaling the amniotic fluid or the liquid. So through that also the infection can get spread. And also second one, intrapartum, the most important thing that we encounter, that is the babies catch the infection through the maternal secretions like for example if it is a cesarean section through the blood and many particles and if it is a normal delivery though the secretion, the babies can get infected. Next is post partum, we see in babies who get discharged and coming back again. So this happens if the mother is not taking proper precautions and if she is not feeding the child and not caring for the child properly. So this mainly happens from the mother or the caregivers in the home, mainly through the respiratory secretions.
Hospital admission & CT Scan in Covid +ve child | Dr. Nilesh Rao
"Those children who have mild to moderate covid infection can be managed conservatively without going to hospital provided we closely manage the hydration and manage the urine output well. Also we need to manage persistent infection like recurrent fevers persist infection and loose stools, reduced urine outputs. Also reduced are CT scans required n children? the CT scans are not required in vast majority of children with covid 19 infection. CT scan exposes children to unnecessary radiation. So it is recommended we seek the pediatricians advice and as per the pediatricians advice in selected cases only imaging and tests may be required"
MIS-C in Children? Can this be detected early? I Dr. Nilesh Rao
"MISC refers to Multi System Inflammatory Syndrome in children. This is a remote complication of COVID 19 infection in children. That is typically seen 4 to 6 weeks after COVID 19 infection in children. This is characterised by a group of systems which can make the child extremely sick. Here the child may develop multisystem involvement of the body that includes the involvement of the brain, heart, blood vessels, clotting system and the kidneys. So the children may get sick. Typically what is seen is that the RTPCR test is negative but the COVID antibody test is positive in these children. This can be detected early if the parents are aware of the typical symptoms associated with MISC. In this case there might have been a COVID history in the family, the child may or may not have been symptomatic and then after 4 -7 weeks child may have presented new onset fever which may be high fever which may be persistent and also other symptoms like recurring vomiting, loose stools, extensive rashes all over the body and swelling and redness in hands. Sometimes a pale grey colour or bluish discolouration of skin and reduced urine output sometimes involving rashes and eye redness. Whenever there is a doubt of child getting sick it is advised to get advise of paediatrician. This is because it is seen if early medical intervention is given in MISC it has better outcomes. Treatment may include steroids and immune suppressing drugs and vital stabilisation"
