Are tests for genetic disorders accurate in Multiple Pregnancy?
"We screen all pregnancies at around 11 to 13 plus 6 weeks of gestation for what we call as the chromosomal abnormalities and the factors what we use for the screening is the maternal age and the scan what is called as the NT scan, which is done around this time, along with the biochemical test, what we call as a biochemical screening test. In twin pregnancy, the most accepted test for Downs is a combination of the maternal age and the NT scan, because the biochemical scan cannot alone be used because of the variation of the level of these markers in the twin pregnancy. However this can be included with the maternal age and the NT scan to increase the accuracy and one thing is under normal circumstances, that is the singleton pregnancies, the age limit or the risk of Down’s is about 35, whereas in twin pregnancy, it is a little early, about 32 to 33 years"
Can Multiple Pregnancy affect the risk of Postpartum Depression? | Dr. Shobha Venkat
"Multiple pregnancy can contribute little more increased incidence of post-partum blues or the depression, maybe because of the lactation problems or the lactational challenges and handling the two babies or the lack of rest, lack of sleep and also anaemia which makes them feel very tired. All these factors contribute to a kind of feeling low or the blues or depression"
Can one breastfeed if having multiples? | Dr. Shobha Venkat
"Breast-feeding is mandatory and should be encouraged. Definitely in all the pregnancies and also in the multiple pregnancies, unless it is contraindicated unless it is discussed by the doctor in person. The only thing is feeding frequently two babies may be a little bit more challenging and a good family support and also addition or adding one or two top up feeds along with the breast feeding will definitely help her continue with her breast feeding"
Can one have vaginal births with twins? | Dr. Shobha Venkat
"Mode of delivery is always an important topic of distension in twin pregnancy. Coming to the place of delivery, it is very important that the twin pregnancy care and delivery should also be in a centre where there is 24 hours facility for senior obstetrician, blood bank facilities and NICU centres and immediate facility for doing the caesarean section. Vaginal delivery is possible for twin pregnancy in the absence of other obstetric indications for the caesarean section and vaginal delivery can be considered if both the babies are with the head down, that is the cephalic presentation or the first baby in the cephalic presentation and the second baby in the breech presentation, provided that the second baby is not too big compared to the first baby and the chances of the caesarean section or the section is a little more in multiple pregnancies and the indication for the caesarean section would be like for any other obstetric indications. Exclusively for the twins the caesarean section is done if the first baby is in the non-cephalic position or in case of monochorionic monoamniotic twins or in the case of complications like TTTS or in conjoined twins"
Does Multiple Pregnancy increases the risk of Gestational Diabetes? Dr. Shobha Venkat
"It has been noted that the gestational diabetes, the incidence is a little more in the multiple pregnancy, probably it is because the multiple pregnancy is seen in a little elderly age group and diabetes is also a problem, which is age related. That could be the reason"
Does one need to gain extra weight if pregnant with multiples? Dr. Shobha Venkat
"In general the weight gain will be little minimal or sometimes it may be even there may weight loss in the first trimester because of the excess vomiting and the less intake of the food. But subsequently what happens is there will be a good weight gain, roughly about 2 kgs every month or half kg per week This is the standard weight gain for singleton pregnancy. But in multiple pregnancy, weight gain may be a little more than that because of the extra retention of the water in the mother, increased blood volume in the mother and because of the two fetuses where which both babies contribute to the weight of the mother"
