We all know that covid pandemic all across the world has kept us in this wild fire.. Covid is not uncommon in newborns So we see many newborns positive immediately after birth. So this can happen if the baby gets positive before birth, or during or after birth. So I am going to discuss about this later. Newborns get infected with covid in cases they are totally asymptomatic and also few babies are having very mild symptoms or moderate symptoms. There are also few case reports saying few newborns have severe illness in the form of ICU admission requiring ventilator support. The newborn can get infected with covid in 3 modes, one is before birth or during birth or one is after birth . what we call in medical terms is intrauterine infection or intrapartum during the time of birth or post partum or after the delivery. So usually what will happen is intrauterine in the sense from the placenta the virus can pass from the blood vessels from the mother to the baby and that can infect the baby and we also have reports that suggests that the liquids that surround the baby inside the womb, it can also contain viral particles normally baby keeps swallowing and inhaling the amniotic fluid or the liquid. So through that also the infection can get spread. And also second one, intrapartum, the most important thing that we encounter, that is the babies catch the infection through the maternal secretions like for example if it is a cesarean section through the blood and many particles and if it is a normal delivery though the secretion, the babies can get infected. Next is post partum, we see in babies who get discharged and coming back again. So this happens if the mother is not taking proper precautions and if she is not feeding the child and not caring for the child properly. So this mainly happens from the mother or the caregivers in the home, mainly through the respiratory secretions.

We need to do covid tests in the newborn also. If the mother is tested positive or suspected to have covid then we need to do the covid tests in the newborn. The second scenario is when there is a cluster outbreak. That means any body in the family or anybody staying with the baby got infected thrid is when the mothers covid status is unknown and suddenly the newborn comes to the NICU, with few symptoms like hurried breathing, fever. So in these cases we need to tests for COIVID. So the re is a gold standard test, what we do is RTPCR . the tests includes the nasal or the oral swab. So we take 24 or 48 hours after birth and we do the test.

The most common type of anesthesia used in obstetrics and gynecological procedure is the spinal anesthesia. The other form of anesthesia used is epidural and general anesthesia. The spinal being one of the most widely practiced is the safest anesthesia and it is a type of regional anesthesia and it tends to numb the nerves that carries the pain stimulus. In case of cesarean section the mother is conscious and awake during the entire procedure and gets to know the entire procedure and sees her baby immediately after she is born. For epidural it is a regional anesthesia very similar to the spinal but here we have an epidural catheter attached to the back and to manage the pain during the full length of the labor. The general anesthesia is less preferred during the cesarean section . Whereas it is used in laparoscopy and short surgical procedures. Here the patient is completely conscious and not aware of what is going on and once the procedure is over the patent is conscious usually in few minutes.

This is a very common question that many mothers ask the pediatric and and the gynecologist. If they are infected with Covid, how long they need to be isolated. Current guidelines clearly say that if the mother is locally asymptomatic, sometimes we electively do test s before the surgery. So if the mother is totally asymptomatic from the day of tests, she has to be isolated for 10 days. After 10 days she has to come pout provided if she is going out and carrying the baby, then better to maintain good handwash and good respiratory hygiene if necessary. If the mother has any symptoms of covid, then in that situation for threw day of the last symptom, if the mother had fever, cough , cod and it lasted for 2 to 3 days, without any medications she did not have any fever form the 4th day, than from that day she has to maintain minimum 10 days of isolation. So the mother should be totally afebrile without taking any fever medications, from that time she has to be isolated for the next 10 days. So before coming out and mingling with other family remembers and going outside.
