"Firstly what is preterm labour? Generally any kind of delivery which happens before 37 weeks is taken as a Preterm labour. But if at all it happens before for 32 weeks that's when we say that it's Borderline pre term and if it is less than 28 weeks of your complete 40 weeks of pregnancy that's when we say that it's an extreme pre-term labour. So when should you be worried that you might be going into pre-term labour is if there is any kind of a repeated pain in your lower abdomen where you feel that there is a complete tightness. Together with this if there is any kind of a increased vaginal discharge which is almost like a fluid leaking outside or if there is any bleeding that is unusual during your second and third trimesters. These are the general signs and symptoms that you might be probably going into a Preterm labour. There are some women who are very high risk towards a preterm labor those people who are very stressed out, who are going through undue physical strain, who have previously had a preterm labour as well as multiple pregnancies, that is twins and triplets. Apart from this medical conditions like diabetes mellitus as well as hypertension can also cause Preterm labour pains. MULTIPLE PREGNANCY Exclusive Pregnancy Care I Dr. Samatha Kumar
"Multiple pregnancies twins and triplets the antenatal care is more frequent antenatal visits with the doctor more frequent ultrasound to monitor the growth of the baby. So in the first trimester we usually ask for a scan to confirm pregnancy between 7 to 8 weeks of pregnancy. Then a scan between 11 to 14 weeks is the month which will diagnose whether it is a monochorionic twins or a dichorionic twins. If it's a dichorionic twins after the first trimester screening is done we usually call them at around 16 weeks to get an cervical length scan which will pick up if there is any shortening of the cervix and if required we might put the cerclage stitch for her. In monochorionic twins along with the cervical scan 16 week scan we can pick up Twin to Twin Transfusion Syndrome also then we do a TIFFA scan in 20 weeks and then every 4 weeks we do scans pick up how the babies are growing. Along with this we ask the mother to take iron and calcium supplements regularly. We advise the mother to monitor the blood pressure regularly at home so that we can pick up eclampsia at a early stage. We screen the mothers for gestational diabetes by doing an OGTT between 24 to 28 weeks and yes in Twin pregnancies from 28 weeks we would call them every 2 weeks for regular antenatal checkups. In dichorionic twin pregnancy we can wait up to 37 to 38 weeks for delivery and then monochorionic twins from 36 to 37 weeks we can plan delivery"
How are SIAMESE or CONJOINED TWINS formed? I Dr. Samatha Kumar
"So twins are formed by either the fertilized egg dividing into two babies or two eggs fertilized separately by the sperms. So where the fertilized egg is divided, they're going separated in that two identical twins formed. In conjoined twins they don't separate completely. There is a union between the twins partially. The union can be at the chest, can be at the abdomen, or it can be at the pelvis. Depending on the type of union we give different names for this conjoined twins. Many of conjoin twins do not actually survive till delivery. But few are born but they are conjoined they have complications sometimes they can even share their internal or. So based on how much of internal organs are shared. They can undergo surgery, get them separated and there are few successful surgeries perform on conjoined twins"
